Hampton Roads Community Health Center (HRCHC), formerly known as Portsmouth Community Health Center, Inc., opened its doors as a federally qualified health center on May 14, 1995, in Portsmouth, Virginia. In 2001, Portsmouth Community Health Center, Inc. expanded its services to Norfolk, VA, with the launch of its Park Place Family Medical Center, located on the corner of 35th and Granby Streets, directly across from Lafayette Park and the Virginia Zoo.
