HERE ARE A FEW NEW PATIENT FACTS THAT YOU SHOULD KNOW…

• We accept various commercial insurances
• We accept TriCare, Virginia Medicaid and Medicare

IF YOU DO NOT HAVE MEDICAL INSURANCE, YOU’LL NEED TO BRING:

• Proof of income:
  • Form 1040: Valid through April 15 of the following year of the tax year on the form. 
  • SSA Award letter: Valid through December 31 of the benefit year listed on the form.
  • Social Services SNAP letters: Valid through the eligibility ending date listed under Number 1 on the letter.
  • VEC W-6 form: Valid for 3 months past the stamped date on form by the employment commission. *Only accepted if no other form of POI is available*
• A government issued identification card
• A social Security Card
• A copay (a nominal fee will be assessed at the time of appointment)
  
  

IF YOU DO NOT HAVE DENTAL INSURANCE AND YOU'RE BEING SEEN AT EITHER OF OUR DENTAL LOCATIONS:

• You need to get pre-registered (Monday-Thursday) from 2-4 p.m.
• Proof of income:
  • Form 1040: Valid through April 15 of the following year of the tax year on the form. 
  • SSA Award letter: Valid through December 31 of the benefit year listed on the form.
  • Social Services SNAP letters: Valid through the eligibility ending date listed under Number 1 on the letter.
